Elderly dog bad breath

Hi, I have a 14 yr old pug poodle cross who has really bad breath. He doesn’t appear to have dental issues now, he has had extractions in past and a dental clean up 18 months ago. I add plaque off to his food daily. Are there any suggestions for something to help please?
 
I give my dog a fish skin flattie every day, it takes him about 15 minutes to chew, and that seems to keep his teeth clean. They say it needs about 15 minutes of chewing to do any good.

 
Not since last time he had treatment, although the vet did say his mouth was pretty good the last time which surprised me because the main reason I took him then was for smelly breath. When he had extractions he was pawing his mouth a lot and he isn’t doing that, eating fine and apart from usual old dog behaviour of sleeping a lot more is a happy little thing.
 
How long has it been since he's had bloodwork done? If it's been more than a few months I'd get him back in to the vet & have a senior panel run, specifically to check his kidney function (bad breath can be a symptom of kidney disease in dogs, and at his age it would not be surprising if his were not operating perfectly any more). Obviously, also have the vet give his teeth/mouth a good exam at the same time, too.
 
There could also be some gastric issues going on. If your Vet can't come up with a reason, I would go for a second opinion.
