Can you tell us a bit more about your product? And why, specifically it is aimed at small dogs?
My dog is 8 kilos, I'm not sure I've come across many things (apart from clothes) aimed at dogs on the basis of size.
She is very young, like a baby so don't expect too much from her. We have a page about toilet training that you may find helpful but do ask if anything is unclear, or you think doesn't apply.

There are other pages that may help too -...
She sounds really smart, so will you give letting her sleep in your room a try? You will soon know if that's the solution. If you don't want it to be permanent we can suggest ways to wean her back to another room, but it would be helpful to know if this works first.
It sounds more like an anxiety issue than a toileting issue then - as the others have said, having her in your room at the holiday place gave her the security to sleep through. Can you have her in your room at home?

With scan as you go, the items go straight into the bag and that's it.
This may not be what you want to hear, but as the breeder you are, and always will be, responsible for the lives you have created. So yes, you must take back this pup.
Whether she is entitled to a refund probably depends on your contract - does it cover returns, and if so does it specify...
Was mum definitely the mum? Interacting with the puppies, enlarged teats etc?
Anyway,
Perhaps a behaviourist might be able to add more value and since there's kids at risk I'd really recommend that. However, it is an unregulated industry, my 90 year old neighbour who has never owned a dog...
When she bites, do you think it's trying to engage you in play, or is it more like a 'driving you away' bite?
You said it comes out of nowhere but there will be something - maybe something barely noticeable - that precedes it. Is there anything at all you can think of?
