Need advice as we are worried

My chihuahua is 13 years old and we have noticed that one of her nipples is quite swollen it’s never looked like this before could this be something serious

The only advice we can give is go and get a vet check. Only they can tell you what it is.

It could be, definitely worth showing the vet. She may have just caught it on something outside but it could be more serious.
Is she spayed?

No she’s not sprayed

This makes it more likely to be serious I'm afraid.
My first dachshund developed mammary lumps at 13 yrs old, they were cancerous. After surgery to remove them she lived very happily for another 2 years.

I would let your Vet see that.

In an elderly, unspayed bitch, anything unusual on the mammaries is to be taken seriously.

It may not be sinister, but you need to be sure.
