Penny went to a veterinary ophthalmologist yesterday. She (the vet) was absolutely lovely, the practice is great, they let me do the restraining which makes all the difference (though I have a suspicion my regular vet put a note in Penny's files that she does better with owner holding her, also much appreciated).
Anyway, Penny was a star. They measured her tear production which involves sticking a piece of paper in her eye and holding it for a minute (picture below), numbed her eyes with drops, put the contrast in, measured the pressure, examined all around her eyes with two different machines, it was all very involved and thorough. Everything is good except for the cloudiness which is basically her cornea is inflamed, aka Keratitis.
Vet strongly suspects it's an autoimmune condition - not Pannus like you see in GSD's, there's no color chance and her third eyelid is not involved, just the cloudy cornea.
Treatment is steroid drops and immunosuppressant drops. The hope is to wean her off the steroid drops soon-ish, and then just the immunosuppressant drops twice a day, that will be for life. Which is fine, it takes less than 5 seconds and she doesn't mind in the least.
She goes back in two weeks to see how she's progressing.
