Random yelping and occasional small spots of blood, possibly issue with paw - Any ideas what to ask the vet

Wonder if perhaps something might be broken in that paw. Ask Vet see if he or she might consider an X-ray. Something is certainly not right.

I've begun to wonder that myself, he'll have a scratch, yelp, then he walks around for what feels like ages keeping that paw way off the ground, I'd thought it looks like he wants to continue scratching and isn't sure what to do, but maybe there's another reason. I'm trying to get this behaviour recorded (failing so far) so the vet can see what he's doing.
